Health Checks and Assessments

45–49 Year Old Health Check

The 45–49 Year Old Health Check is a one-off check-up designed to assess your risk of developing chronic disease in the future. It provides an opportunity to review your lifestyle, medical history, and family history, and to evaluate your risk factors for chronic conditions.

Health Assessment for Older Adults

The Health Assessment for Older Adults is an in-depth annual assessment for patients aged 75 years and over. It provides a structured approach to identifying health issues and conditions that may be preventable or responsive to interventions, with the goal of improving overall health and quality of life.

These assessments are conducted by our experienced nurses and typically take around 1 hour. During the assessment, you can expect discussions about:

  • Functional assessment – including mobility, balance, falls risk, continence, hearing, and vision

  • Cognitive screening – to check for early signs of memory loss or dementia

  • Mental health review – including mood and social engagement

  • Social supports and safety – including living arrangements and care needs

  • Health promotion advice – including nutrition, exercise, and immunisations
